Job Summary

We are recruiting for a Enhanced Care Team Practitioner who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

You will be responsible for the support and management of the team in the absence of the Team manager, undertaking sicknessmanagement, appraisals, assisting with training and ensuring staff competencies are completed and up to date. You will also provide support to patients who have confusion, mental health conditions, Dementia or Delirium and need 1-1 enhanced care and do all personal care for your patients, documentation, distraction therapies, activities, and encourage mobility.

We are looking for an individual who, has excellent communication skills, can work independently, works flexibly and can deal with challengingbehaviour, in a pleasant and calm manner. Previous experience of working with Dementia and mental health patients is desirable.

Main duties of the job

The post holder will be expected to work long days, nights and weekends to ensure the service has consistent senior cover.

Responsible for the leadership and management of the Enhanced care team.

Required to liaise with the senior site nurse and the Safer staffing team in the coordination and placement of the Enhanced care team to ensure patient safety on all wards.

Expected to provide support to patients who have confusion, Dementia or Delirium and need 1-1 enhanced care.

Required to assess and review patients on a shift by shift basis ensuring care plans are up to date and behavioural charts are completed.

Required to move around the hospital to provide care and support to the patients with the greater need. You will be expected to do all personal care for your patients, documentation, distraction therapies, activities, and encourage mobility.

Expected to manage the team out of hours, supporting the team to complete appraisals, training and sickness management.

Person Specification

Qualifications / Training

Essential

  • Foundation Degree in Health and Social Care Practice (FdSc) NVQ level 3 in Care NVQ A1 Assessors Course and Mentorship training

Desirable

  • AI NVQ Assessor

Previous Experience

Essential

  • Previous experience in health care undertaking a full range of clinical competencies to NVQ Level 3
  • Experience with Dementia care.

Desirable

  • Experience in Dementia, mental health, leadership experience

Skills

Essential

  • Ability to communicate in a variety of settings with patients of varying levels of understanding
  • Ability to liaise with all members of the multi-disciplinary team
  • Ability to work on own initiative within protocols and remit of role, with minimal direction by a registered practitioner
  • Ability to undertake basic aspects of assessment, planning evaluation and delivery of care. Assessment skills and ability to problem solve
  • Ability to prioritise care and organise workload. Self motivated and able to work on own initiative
  • Understanding of quality and change in the clinical setting
  • Flexibility and team participation

Knowledge

Essential

  • Understanding of the requirements of the role of Associate Practitioner in the context of the Team and the Organisation
  • Provide evidence of managing continuing personal development and being proactive in development of self and others.
  • Risk management and Infection Control Knowledge of clinical observation
  • Understanding of NHS issues

Desirable

  • Experience of supervising, training and developing clinical support workers
