Job Summary

The postholder will support the ongoing implementation and delivery of Personalised Care for people affected by cancer, specifically carrying out holistic needs assessment (HNA’s) with patients and producing associated care and support plans.

Main duties of the job

To support the successful implementation of holistic needs assessments (HNA’s) for people affected by cancer across the NCA, assisting with the process of offering and delivering HNA’s to patients and producing associated, comprehensive care and support plans

Provide a single point of access, including rapid re entry into the system for those people identified as having urgent or specialist needs

  • Providing health and well-being information and support in relation to their cancer, signposting to supportive services to enable navigation of health and social care; to empower the patient to manage their cancer.
  • Work closely with the Macmillan Information & Support Service to ensure appropriate referrals to the service are made.

Person Specification

Qualifications

Essential

  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent
  • Qualification in healthcare related subject / equivalent knowledge and experience at Level 3 or above (e.g.: Health and Social Care / Diagnostics and Therapeutics
  • High standard of English and an ability to convey information accurately, succinctly and in a sensitive manner both verbally & in writing.

Desirable

  • Communication skills training

Knowledge, Training & Experience

Essential

  • Excellent communications skills with experience of dealing with challenging / distressing situations.
  • Able to provide high quality and accurate written information.
  • Understanding of personalised care and the importance of this
  • Excellent organisation, planning and time management skills.
  • Demonstrable IT skills including Microsoft Word, Excel, PowerPoint

Desirable

  • Experience of providing support to people affected by cancer.
  • Experience of undertaking holistic needs assessments
  • Knowledge of cancer, cancer treatments and cancer pathways

Skills & Abilities

Essential

  • Good interpersonal and communication skills (written, oral and electronic)
  • High levels of empathy and ability to deal with situations compassionately.
  • Good organisation skills and attention to detail and problem solving skills.
