Job Summary
The post holder will be a member of the Intensive Community Support Team (ICST) within the Community Adult Learning Disability Team.
They will provide specific skilled support to aid the ICST in the assessment and management of adults with a learning disability, requiring behavioural support within the community.
They will help support with training to service providers and carers across the Bexley, Bromley and Greenwich.
The post holder will be expected to facilitate group work with patients and carers.
They will share a caseload with the Clinical Nurse Specialist and will be expected to work unsupervised at times.
Main duties of the job
The Specialist Support Worker will work as part of the Intensive Community Support Team with a shared caseload of people with Learning Disabilities and challenging behaviour who live in the community who are at risk of home/placement breakdown.
With Nursing supervision, work alongside Psychology, Occupational Therapy, Speech and language therapy, and Positive Behaviour Support Practitioners.

- Under the guidance and supervision of the Clinical Nurse Specialist, the post holder will be expected to hold a small caseload of patients with learning disabilities, taking responsibility for implementing and reinforcing interventions.
- To work with patients, family and carers individually or in a group.
- To engage and participate in assessments
- Information gathering through liaison with patients, carers and the MDT.
- Monitoring an individuals mental health/behaviour/well-being.
- Contributing to clinical and non-clinical reviews, this may include obtaining vital signs (blood pressure, temperature, pulse, heart rate, respiration rate) for which, training will be provided.
- Liaise with wards to support care delivery, where appropriate.
- Attend multi-disciplinary meetings to support an individuals care and management.
- To attend local team meetings including attending referrals meetings as and when required.
- To support the CNS (Clinical Nurse Specialist) in the delivery of learning disability training as appropriate.
- Be computer literate and to be able to use the trusts record keeping system and use documents such as Microsoft Word, Excel, Teams etc.
- To provide specialist support with patient appointments if/when necessary.
- Have a good understanding and awareness of the rights of patients and carers and assisting them in exercising those rights where appropriate.
