Job Summary
An exciting permanent, full time opportunity has arisen to join our Older Person’s Medicine Physiotherapy Team as an Associate Practitioner, this post is predominately based at the Freeman but would include occasions where cover would be provided to the RVI team also. Our wider MDT team compromises medical, nursing, occupational therapy and physiotherapy clinicians. Our MDT is a welcoming and collaborative group who treat a wide variety of conditions and use different treatment approaches to ensure our patients receive person centred care.
Main duties of the job
Assessment and provision of 1:1 therapy programs under supervision of a qualified therapist for a variety of conditions. These conditions include:
- Sarcopenia and Frailty
- Mobility and Gait Rehabilitation
- Balance / Vestibular Impairments
- Joint / Orthopedic Conditions
- Dementia / Mental Health conditions
Non-clinical duties include:
- Documentation for group and individual therapy sessions
- Complete discharge summaries for patients participating in groups
- Complete referrals and signposting as required to other services ( e.g., podiatry, community exercise groups)
- Ordering of equipment as required for team
- Data entry for audit purposes to review group effectiveness
- Complete quality improvement projects
- Participate in team in-services
- Communicate and coordinate with team members for allocation of clinic duties

- Knowledge and understanding of clinical conditions and application of Physiotherapy Rehabilitation programmes for patients within speciality area
- Knowledge of physiotherapy outcome measures

- Highly developed communication skills

- Foundation Degree or NVQ level 4 or NVQ level 3 with additional training in the speciality field or equivalent evidence of underpinning knowledge
