Job Summary

An exciting opportunity has arisen within our dynamic team to work with people who have experienced Stroke or Brain injury as they return home from hospital and rebuild their lives. The successful applicant will work with a range of professional colleagues including occupational therapists, speech therapists, psychologists, a dietitian, and physiotherapists to support individual rehabilitation programmes with people in their own homes and communities. There will be opportunities to support the delivery of our Niwrostwt Neurological Recovery College programme and online groups. Applicants should have a level 3 qualification in a relevant field and will be required to undertake the AGORED level 4 Therapy Assistant Practitioner practice-based Qualification.

Main duties of the job

To contribute to the provision of health care to the people of Gwent by providing high quality, effective support and rehabilitation services to people who have had a stroke or brain injury enabling people to return home from hospital early and rebuild meaningful lives. This activity supports the ABUHB Clinical Futures work reducing length of hospital stay, providing care closer to home and establishing home as the hub.

The post holder will carry a delegated caseload of people with stroke and other neurological conditions and will work closely with registered clinicians within the service.

Person Specification

Qualifications and/or Knowledge

Essential

  • Evidence of AGORED Level 3 qualification relevant to workplace or equivalent appropriate support worker training / qualifications
  • STARS stroke training or equivalent
  • Understanding of the complex nature of Rehabilitation.

Desirable

  • ECDL
  • Advancing STARS stroke training
  • Knowledge of a range of rehabilitation interventions
  • Knowledge and application of Microsoft Word, and Outlook

Experience

Essential

  • Experience of working as a support worker or in an allied field. Experience of managing own time and prioritising activities.
  • Experience of self-development demonstrated by CPD portfolio. Experience of contributing to improving services
  • Experience of working independently

Desirable

  • Previous experience as an OT/physio/SLT Assistant or HCSW
  • Experience working with people following stroke or brain injury
  • Experience of working in a variety of teams
