Job Summary

The purpose of the Lung Health Screening Programme is to enable the early detection of lung cancer diagnosed at Stage 1 and 2 to improve survival rates for those patients diagnosed with cancer. The Lung Cancer Screening Programme targets those most at risk of lung cancer. The Team offer Lung Health Checks to patients between the ages of 55-75 with a history of smoking This role will also work with the Lung cancer team to provide support and advise people in accessing appropriate information, by sign-posting to a variety of resources. Promote and educate towards a self-management approach, where appropriate. Contribute to Holistic Needs Assessments and development of an individual Care Plan in line with practitioner guidance, protocols and procedures.

 

Main duties of the job

  • Carrying out Lung Screening Health Check assessments with patients
  • Carrying out baseline observations
  • Use of IT systems Soliton/Infoflex/ sunrise and any other system that is required
  • Working closely with clinical team to support patient flow and positive patient experienceo Booking of appointments.
  • Contribute to Holistic Needs Assessments and development of an individual Care Plan in line with practitioner guidance, protocols and procedures.
  • Be able to refer and sign post to the clinical team as appropriate for complex decisions
  • Have oversight and support with the PTL to ensure patients are meeting the 28 day (FDS) and 62-day standards for cancer. Escalating any issues or delays to the Lead Lung CNS.
  • Able to risk assess patients on an individual basis, appropriately refer on to other services, able to triage patients with escalation process within competencies
  • Communicate effectively with relevant MDT’s and clinical teams to ensure that all appropriate cancer patients are identified for co-ordinated support, as assessed by health professionals
  • Support and advise people in accessing appropriate information, by sign-posting to a variety of resources. Promote and educate towards a self-management approach, where appropriate.
  • The role will require liaison and effective communication with all members of the multi-disciplinary team, patients and carers to provide co-ordinated high-quality patient-centred care.

Person Specification

Qualifications

Essential

  • GCSE’s Grade A-C -Including English and Maths, A Levels
  • Relevant health or social care experience at Band 3 or equivalent
  • Courses/further study attended to demonstrate evidence of continued personal development

Desirable

  • NVQ Level 3 Administration or Social Care
  • Educated to diploma level or working towards e.g. good knowledge across a range of health or social care settings

Skills and Experience

Essential

  • Previous experience in a health service background
  • Previous experience in a health service background
  • Able to work on own and within a team using own initiative.
  • Range of IT skills and use of hospital systems to support patient care and data input. Proficient in the use of Microsoft Office
  • Experience of dealing with non-routine and complex problems and progress chasing
  • Proven ability to work to deadlines, prioritise and multi-task

Desirable

  • Experience of using a radiology information system
  • Experience within respiratory medicine or oncology
  • Previous experience of using Infoflex
  • Experience of coordinating a patient workload
