Join us as a Band 4 Therapy Technician in the Intermediate Care – Community Bridging Service, where you’ll work both independently and as part of a supportive team to deliver high-quality rehabilitation, assessment, and care to patients in their own homes.

You’ll play a key role in promoting patient independence, supporting individualised goals, and working closely with registered practitioners to deliver effective treatment programmes. If you have a foundation degree or NVQ Level 5 (or are working towards one), experience in health or social care, excellent communication skills, and a passion for helping others, we want to hear from you!

Main duties of the job

  • To work as a proactive member of the team providing a high quality, efficient and effective service to patients.
  • To work autonomously, assuming responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction and supervision of a registered practitioner. To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.
  • To work with patients in their own homes providing assessment, rehabilitation, support and treatment programmes, undertaking home visits as appropriate. Working as a trusted assessor undertaking initial assessments and functional assessments as delegated by registered staff and supporting clinical staff with treatment and rehab input. To work within scope of competence.
  • To be proficient, knowledgeable and competent in rehab and functional assessment. Educated to Foundation Degree, Level 5 NVQ in Health and Social Care or be able to evidence that they are working towards one of these qualifications. To continue to develop in the role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.
  • To have a vehicle for use for work purposes and have business insurance. To work as part of a rota covering the Trust’s Intermediate Care Service 08:00-20:00 7 days a week.

Qualifications

Essential

  • Foundation degree, NVQ Level 5 or evidence of working towards one of these qualifications, relevant to this role

Desirable

  • Progression of patient treatment

Experience

Essential

  • Previous experience as an HCA, Community Support Worker or Therapy Assistant

Desirable

  • Competency in performing baseline observations.

  • Good communication skills to include concise and effective verbal and written reports
  • Ability to follow clear instructions and feedback to senior staff
  • Able to work with minimal supervision
  • Previous experience undertaking assessments and rehab
  • Able to motivate people and monitor progress
  • Good time management
  • Ability to cope with and analyse changing situations
  • Prioritisation skills
  • Confident computer skills with word and patient record systems

Desirable

  • Teaching skills
  • Knowledge of risk assessment
