Job Summary

This is an excellent opportunity for an experienced Therapy Assistant/Support Worker to join our dynamic transfer of care therapy team.

You will have experience of working with integrated therapy services. The role will support transfer of care and the acute therapy teams to ensure timely discharges of patients from the acute setting, supporting a home first approach and reducing demand for the intermediate care settings. The successful candidate will be organised, an effective communicator and strong team player.

You will work within an integrated model of care to provide therapy within the acute setting, optimising function and support with discharge planning for our patients.

We are looking for individuals who work well as part of a team and can contribute to innovative ideas on clinical pathways and can help drive forward excellence.

You must be adaptable to working with a range of patients and working environments with a flexible approach to the delivery of care. This must include a willingness to adopt working patterns that include weekends and public/bank holidays which may form part of your contracted hours or may include working additional hours.

 

Main duties of the job

Support will be provided by the integrated therapy team who will take a proactive and innovative approach to continuous professional development, evidence-based practice and service development.

In addition, you will carry out a range of non-clinical duties related to the role.

Ability to work over 7 days, you will be expected to be flexible in terms of working patterns including weekends, bank holidays and extra hours if necessary.

Qualifications

Essential

  • Foundation degree Health and Social Care, or equivalent level 5 qualification; plus, A-levels and GCSEs or equivalent(s), including Functional Skills (level 2) Or – as a level 5 apprenticeship, such as the Assistant Practitioner apprenticeship Care Certificate
  • Care certificate

Knowledge, Training & Experience

Essential

  • Substantial experience in health, therapeutic, rehabilitation and or social care setting
  • Understanding of evidence-based practice and up to date knowledge of relevant literature
  • Evidence of therapy skills competencies
  • Evidence of clinical skills competency

Skills & Abilities

  • Ability to communicate effectively with people of all ages and abilities
  • Effective organisational skills
  • Ability to demonstrate empathy, to motivate and enable and to be sensitive to diverse individual needs
  • Ability to manage own caseload and work without direct supervision
