Job Summary

The Community Response Team supports discharges from the acute hospitals, providing short-term and interim care. The team will also provide re-enablement to suitable patients to improve independence. Previous caring experience is ESSENTIAL.

You will be based at Babington Hospital in Belper to cover the localities of Amber Valley and Erewash. You will need to be able to travel between visits, therefore a full UK driving licence and access to a vehicle is essential.

Shifts follow a 7 day rota, including weekends and bank holidays.

Main duties of the job

  • To be responsible for the day to day supervision of Support Workers within the Community Response Team.
  • Delegate patient visits to ensure an effective support worker service is provided to patients in a timely and cost effective way.
  • Accept new patients on to the caseload and be able to complete new patient assessments as per DCHS standards.
  • To be able to set up and modify care plans for the package of care and re-enablement support required.
  • To be a role model to the Support Worker team and actively monitor and maintain clinical skills and professional manner.
  • Communicate effectively with patients and carers to maximise rehabilitation potential as appropriate and under supervision.
  • Work with qualified Nurses and Therapists in the assessment and treatment of patients with a variety of conditions and progress/modify intervention programmes within parameters set by the Therapists and Nurses.

Person Specification

Education, qualifications & training

Essential

  • NVQ Level 4 or able to demonstrate equivalent knowledge gained through experience and training
  • GCSE English Language and Mathematics (or equivalent)
  • Good knowledge and understanding of rehabilitation /delivery of care
  • Recent and relevant experience of working within adult/older people’s rehabilitation setting
  • Experience working as Band 3 Support Worker/Healthcare Assistant
  • Community experience
