Job Summary
The Assistant Practitioner post within the community COPD Service will support service delivery and provide patient care in keeping with management plans devised by clinicians.
Patient care will include Pulmonary Rehabilitation, one to one patient education and support on home visits and attending the patient support group.
The post holder will also provide administrative and technical support to the service.
Main duties of the job
- Work alongside the Physiotherapist to deliver Pulmonary Rehabilitation programmes and patient assessments associated with this.
- Attend the patient support group “Breathe Easy” and contribute to the running of the group/schedule.
- Undertake selected home safety checks for home oxygen users and feedback findings.
- Ensure that management plans are implemented and report back to clinicians.
- Undertake basic clinical duties such as observations and other tests when deemed competent to do so
- Deliver patient education for example inhaler use/technique. Smoking cessation and support.
- Liaise with members of the wider team including Social Services, Prevention Alliance and Crisis Response team
- Educate patients to encourage self-management.
- Support patients with complex needs to remain at home via delivery of a specific management plan.
- Assist within the MDT specialist clinics.
- Undertake phone calls, taking referrals and organising patient care.
- Facilitate discharges following the plan from the clinician.
- To be an ambassador for the COPD service and demonstrate quality care for our patients.
- Comply with FT policies for personal and patient safety and for prevention of healthcare-associated infection (HCAI); this includes a requirement for rigorous and consistent compliance with policies, procedures and guidelines in relation to all aspects of infection control
