Responsibilities

• Providing personalised care and support to clients in their homes or community environments
• Assisting with daily living tasks such as personal hygiene, meal preparation, and medication reminders
• Developing and implementing care plans tailored to individual needs in collaboration with healthcare professionals
• Supporting clients in maintaining their independence and participating in community activities
• Driving clients safely to appointments, social activities, or errands as required
• Maintaining accurate records of care provided and any changes in client condition
• Communicating effectively with clients, families, and healthcare teams to ensure consistent and high-quality care

Requirements

• Valid driving licence and the ability to drive safely within the community
• Experience working in a care home or providing home care services is desirable but not essential; training will be provided
• Knowledge of care plans and ability to follow prescribed routines
• Proficiency in English, with strong communication skills to engage effectively with clients and team members
• Basic IT skills for documentation and record keeping purposes
• Compassionate nature with excellent interpersonal skills
• Ability to work independently and demonstrate reliability in delivering quality care
