Job Summary
Following a recent transformation of Learning Disability services, we have developed an innovative new role which offers a unique chance to shape how people with a learning disability access and experience healthcare. As one of the first postholders, you’ll have the opportunity to shape and evolve the role, bringing your ideas and initiative to life.
In this rewarding role, you’ll work closely with multidisciplinary colleagues to support individuals, families, and carers in navigating health services. You’ll be a central point of contact, helping people access the right care at the right time, advocating for their needs, and promoting inclusion and independence.
You’ll use your initiative to overcome barriers to healthcare, build relationships with local services, and empower clients through personalised support.

Main duties of the job
The post holder will work within the Community Learning Disability Team under the direction of the team manager, and as part of a multidisciplinary team will support people with a learning disability to achieve better health outcomes and experience of services using a personalised approach.
Navigating health services can be challenging and the postholder will offer support, guidance and actively work with the person, their family and carers to identify and access the most appropriate and expert service to meet their needs effectively. Often working independently and using their own initiative, the postholder will ensure clients are able to access the right care by the right service in a timely manner.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
