An exciting opportunity has arisen for a compassionate and motivated Social Worker to join our friendly and highly specialist Cystic Fibrosis (CF) and Metabolic Bone teams, who is passionate about working with children, young people and their families in delivering high-quality, family-centred care within a multidisciplinary setting.

About the Team

Cystic Fibrosis (CF) Team The CF team provides care for children and young people aged 0-16 years living with cystic fibrosis. The service supports patients and their families from diagnosis through childhood and adolescence, and into transition to adult services. The team works closely with families to address medical, emotional, social, and practical needs.

Metabolic Bone TeamThe Metabolic Bone team delivers specialist care to children with osteogenesis imperfecta (brittle bone disease). The service focuses on holistic, individualised support, helping children and families manage the impact of a long-term condition on daily life, education, emotional wellbeing, and independence.

About You

  • A qualified Social Worker with current Social Work England registration
  • Experienced in working with children and families, ideally within health or specialist services.
  • Confident in managing complex cases and working with long-term or life-limiting conditions.
  • A strong communicator who values team work and partnership working
  • Compassionate, resilient and committed to delivering high standards of care

 

Main duties of the job

Reporting into the Clinical Nurse Specialist (Cystic Fibrosis), some of your responsibilities will include:

  • Provide specialist social work support to children, young people and families affected by long-term health conditions
  • Provide emotional, practical and professional guidance, promoting resilience and wellbeing
  • Undertakeholistic assessments and develop family-centred care plans
  • Work collaboratively within a multidisciplinary team to deliver coordinated, high-quality care
  • Advocate for children and families to ensure their voices inform decision-making
  • Support families through key stages including diagnosis, hospital admissions and transition to adult services
  • Maintain accurate records and contribute to service development and quality improvement

  • Health and Safety
  • Current Social Care practice / issues

  • Specific clinical interventions / strategies
  • Liaison, negotiation and influencing
  • Ability to drive and have access to a vehicle

Experience

Essential

  • Post qualifying significant experience within a CAMHS / Children’s Social Work setting

Qualification and Training

Essential

  • Degree in relevant subject
  • Social work qualification / registration with current registration with Social Work England
  • Post qualifying training relevant to CAMHS or Children & Young people
