Job Summary

This is an exciting Band 4 Speech and Language Therapy Assistant Practitioner post based in Cardiff Community Resource Team, supporting residents of care homes across the city who are living with acquired swallowing and communication difficulties. The role sits within a collaborative multidisciplinary team aiming to keep people well at home and reduce the need for hospital admissions.

 

Main duties of the job

We are looking for a highly motivated,organised and innovative individual to fill our Band 4 Speech and Language Therapy Assistant Practitioner post.The post holder will work as part of a therapy-based team that assesses and treats patients aged 18 and above with a wide range of medical diagnoses. This position will focus on supporting people living in care homes across Cardiff.

The post holder will participate in assessment and management of care home residents within protocols,managing their own caseload and demonstrating clinical reasoning skills. They will access regular clinical supervision and guidance as well as opportunities for continued professional development.The post holder will be a point of contact for care homes and address queries and identified training needs.The post holder will attend regular team meetings and contribute towards service development projects.

A strong, reliable team player is required for this role, who is able to demonstrate excellent communication and interpersonal skills, negotiation, time management,initiative and IT skills. The post holder will work from office bases in Whitchurch and Llanrumneyand in care homes across Cardiff, with some opportunities for agile working.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
