Job Summary

The post holder is responsible for participating and assisting in the delivery of assessment and post-diagnostic support to service users and family carers within the Haringey Memory Service. They will be responsible for a caseload of patients living with dementia in the community and will be undertake regular reviews, offering signposting and delivering low-level interventions such as Assistive Technology, Life Story Work and memory strategies.

They will work as part of the multidisciplinary team to provide excellent, compassionate and sensitive care to people who have dementia and their carers. Working under supervision, the post holder will provide direct support to a defined group of service users as allocated and delegated by the Team Manager or another more senior member of staff.

Main duties of the job

Actively participating in the delivery of post-diagnostic support to service users diagnosed with dementia and their family carers as per the plan of care determined by the multidisciplinary team. Encouraging service users to engage in their individual programme of care and support and with their consent, where appropriate, seek the cooperation of friends / relatives / carers.

To undertake and assist in the review of service users with dementia to include:

a) Signposting service users and family carers;

b) Actively linking service users and family carers in with local and/or national services;

c) Referring service users and family carers to statutory and third-sector organisations;

d) Recording any changes in physical and mental health and reporting these to a more senior member of staff, acting appropriately on the advice given;

To support other members of the team in the diagnostic process through initial data and information collection, arranging reports, liaising with primary and secondary care, discussion and direct service user support.

To take part in Occupational Therapy interventions such as assessing for low level equipment and Assistive Technology under the supervision of an Occupational Therapist.

To take part in psychological interventions i.e. Cognitive Stimulation Therapy or psycho-education as per individual plans of care under the supervision of a Psychologist.
