Job Summary

The States of Guernsey are looking for a Care Coordinator to join the Adult Disability Service.

 

The post holder will act as a Care Coordinator for an allocated caseload of adults with learning disabilities and/or autism within Adult Disability Services. The role involves undertaking holistic, strengths-based, and person centred assessments, care planning, risk assessments, carers assessments, and coordination of support for individuals with a range of complex needs.

The post holder will work collaboratively with individuals, their families, carers, and a wide range of multi agency partners to ensure effective, coordinated, and outcome focused care. They will take a lead role in coordinating care where appropriate, ensuring that individuals are supported in a way that promotes independence, dignity, and inclusion.

The role requires autonomous working within a supportive multidisciplinary framework, with responsibility for managing risk, safeguarding, and delivering high-quality, evidence-informed care.

This role is open to registered health or social care professionals.

Person Specification

Qualifications

Essential

  • Registered Learning Disability Nurse with the NMC
  • Or a
  • Registered Social Worker with Social Work England or a Registered Health or Social Care Professional with HCPC.
  • Significant experience at a Band 6 or equivalent
  • Practice assessor, practice supervisor
