Job Summary

Sussex Partnership NHS Foundation Trust (SPFT) are looking for Band 4 Community Support Workers to provide support to individuals living with Multiple and Compound Needs (MCN) across East Sussex.

The roles will form part of a Multi-Disciplinary Team (MDT) which includes specialists with lived experience and from Adult Social Care, health, mental health, domestic abuse and substance dependency services, working together. Our aim is to provide flexible, trauma-informed, and person-centred support.

MCN is defined as ‘people who experience three or more of: homelessness or housing issues, current or historical offending, substance misuse, domestic abuse and mental ill health’.We are looking for innovative thinkers and those that truly believe in and practise a trauma-informed approach.

 

Main duties of the job

The role is dynamic, and no two days will be the same.Will hold a small, but intensive caseload (between 10-12 cases), supporting individuals with MCN with severe and enduring mental health needs.

The role consists of outreach and therapeutic interventions to individuals living with MCN who struggle to access secondary mental health services and promote partnership working through a ‘Team Around Me’ (TAM) Model.

The postholders will support ongoing work to achieve systems change by identifying blocks and barriers in the system, recognising best practice, and by working flexibly to overcome the challenges facing people with MCN. The post holders with have specific responsibilities for arange of delegated clinical duties in a community mental health setting.

The post holders will contribute to the growth and maintenance of key functions of the clinical area by carrying out assigned tasks, involving direct and indirect care to individuals under minimal supervision and as directed by qualified staff and/or Team Leader. Following personal development planning and targeted training the competent practitioner will be in a position to work more autonomously in supporting clients.

As a result, you will have developed a broad underpinning knowledge base at intermediate level and have the ability to evaluate information and communicate with a range of professionals

QUALIFICATIONS

Essential

  • NVQ Level 3 in Care and equivalent skills acquired through experience

EXPERIENCE

Essential

  • Experience working within a health or social care setting
  • Experience of working as part of a team
  • Good communication skills
