Job summary
To assess and work with mental health service users and signpost them to other services and discharge to primary care where necessary. Any service users needing longer term secondary care and care coordination will be transferred to the relevant teams in the Trust.
The post holder is expected to utilise community resources, working closely with Primary Care.
The post holder will ensure user and carer involvement in all aspects of service delivery.
This is a senior nursing role with immense experience in duty and mental health assessments – it is not suitable for newly qualified nurses.
Working experience in HTT/SPA/Liaison Psychiatry work will be desirable. Those without the relevant assessment and risk management experience need not apply.
The suitably qualified candidate will be able to work a combination of remote working and coming to our office at Springfield hospital in line with the needs of our patients. This will be guided by our Agile working policy which means that the successful candidate must be prepared to, on days when they are meant to work remotely, go on home visit to patients’ homes or come into the office at short notice whenever the service needs require this. This is however a rare occurrence.
The post holder will communicate and collaborate effectively with all agencies and agents involved in supporting adults with severe mental health problems.
Main duties of the job
To ensure that the views of users and carers are sought and incorporated into all care delivered.
To provide clinical cover for team members in their absence.
To liaise as required with other disciplines and organisations to ensure that all issues affecting service user care are managed and communicated to a high standard.
To provide written assessments reports/discharge letters for all allocated clients.
To liaise closely with other services keeping them informed of relevant service issues.
To provide information and reports as required and accurately document all relevant information.
To have an awareness of the need to manage resources effectively within determined budgets.
To assist in the successful induction of new staff and regular supervision of students and junior staff.
To have responsibility for the health, safety and welfare of self and others. To comply always with the requirements of the Health and Safety regulations.
To participate in the development of junior staff, acting as a mentor for student nurses and a preceptor to newly qualified staff, ensuring appropriate teaching, support and supervision.
To participate in clinical/team meetings.
To enter information onto Rio in a timely manner in line with Trust guidelines and prepare reports as appropriate.
Person Specification
Experience & Knowledge
Essential
- Demonstrable experience of working with people who experience serious ongoing mental health problems in a variety of settings.
- Experience of working in a multidisciplinary team
- Experience of assessing and managing crisis with those with a severe mental illness
- oExperience of providing thorough baseline mental health assessments and case formulation based on these
Desirable
- Experience of working in community nursing
- Experience of working in a CMHT or other community health setting
- Experience of contributing to policy and procedure development and implementation
Education and Qualification
Essential
- RMN, social worker or occupational therapist with current relevant registration
- Mentorship qualification
Desirable
- Diploma/BSC in Community Nursing
- Evidence of post registration training related to the provision of relevant psychosocial interventions
Skills & Abilities

- Ability to work as an active member of the multidisciplinary team
- Knowledge of recent initiatives in nursing
