Job summary

We have an exciting opportunity for a
Band 5 Occupational Therapist to join SHAPE (Supporting Health and Promoting
Exercise). The primary purpose of this role is to support the delivery of a health
and exercise programme for patients on the Severe Mental Illness Register. The
role will also support the wider team in ensuring the annual physical health
checks for this population through OT assessment and care planning for those
harder to reach patients.

SHAPE is provided by Herefordshire and
Worcestershire Health and Care NHS Trust to reduce health inequalities and improve
health outcomes by offering patients weekly sessions with advice on health and
wellbeing as well as the opportunity to try activities including Zumba, Pilates
and Pickleball.

This is a full time post working Monday
to Friday. The post holders will be based at Studdert Kennedy House and will
support the delivery of SHAPE in several locations across Herefordshire and
Worcestershire. We encourage applications from people with a background in health

Main duties of the job

The role will
require:

Provision of
occupational therapy assessments, addressing occupational performance and skill
deficit, enabling the client to attend groups and physical health check
appointments

Work with clients
to identify occupational therapy goals as part of the overall care plan, taking
on the role of care co-ordinator where relevant.

Plan and
implement client centred individual and/or group interventions in collaboration
with the client, using graded activity to achieve therapeutic goals.

Monitor, evaluate and modify treatment for
clients with multiple needs in order to measure progress and ensure
effectiveness of intervention.

Supporting
patients in accessing their annual physical health check

Engaging in
regular physical activity

Joint working and
planning with stakeholders and voluntary sector providers and signposting
patients to other services, VCSE and for specialist providers where
appropriate.

Multidisciplinary
team working and communication

Person Specification

Qualifications

Essential

  • Diploma/degree in OT
  • Practice Placement qualification within one year of appointment
  • Registration with HCPC
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Experience

Essential

  • Documented evidence of continuing professional development

Desirable

  • Post registration experience in a relevant clinical setting
  • Experience of research or audit
  • Advice and support of junior staff and/or students
