We are looking for a dedicated and motivated Senior
Rehabilitation Assistant to join our Responsive Services team in this
exciting and rewarding new role.

As a Senior Rehabilitation Assistant, you will play a key role
in supporting patients to achieve their rehabilitation goals and remain
safely at home. Working under the guidance of qualified clinicians, you will
carry out basic assessments, provide equipment, and deliver rehabilitation
programmes designed to maximise independence and improve quality of life.

In addition to managing your own delegated caseload, you will
provide day-to-day support to Rehabilitation Assistants, helping to
coordinate and allocate work across the team.

You will work closely with a friendly and supportive multidisciplinary team, including Physiotherapists, Paramedics, Occupational Therapists, Nurses, Health Care assistants, Rehabilitation Assistants and Administrative Staff. Responsive Services is an admission avoidance and facilitated discharge service, providing short-term rehabilitation and support to people who have recently been discharged from hospital or who are at risk of hospital admission.

Due to the community-based nature of this role and the geographical area covered by the service, applicants must hold a full driving licence and have access to a vehicle suitable for business use. Unfortunately, public transport is not a viable option for carrying out this role.

Responsive Services operates 7 days a week, 365 days a year, between 8:00am and 8:00pm. The successful candidate will work as part of a rota to support service delivery across these hours.

  • Able to work flexibly in order to provide personal care and support to patients, sometimes at short notice

Experience

Essential

  • Previous experience of caring for someone who was elderly, disabled or ill
  • Previous experience of rehabilitation
  • Experience of working within a therapy service or other relevant field –
  • This may include voluntary as well as paid experience
  • Experience of working in a community setting
  • Work independently and as part of a team

Desirable

  • Supervision of staff
  • Experience of using virtual communication platforms, i.e., Skype, Teams, Zoom
  • Contributing to improving services
  • Previous NHS Experience

Essential

  • Good communication skills and interpersonal skills, including an ability to build rapport and establish good one to one relationships
  • Ability to organise own time efficiently
  • Ability to work effectively as part of a team and to maintain a positive team-working relationship by valuing the contributions of other team members, understand others needs, using a flexible approach, handling conflict positively in an open and constructive manner

Qualifications

  • Good general knowledge and education to GCSE standard
  • NVQ 3 or studying towards CQF diploma level 3 in clinical health care support
