Job summary
This is a substantive post created to provide consultant level care to respiratory patients in the community and secondary care with a focus on COPD. The post holder will work with the Integrated respiratory teams to enable care of suitable patients in neighbourhoods and at home. The postholder will be the COPD Virtual ward clinical lead for Imperial College Healthcare NHS Trust and contribute to the acute medicine on-call rota at St Mary’s Hospital (SMH), providing respiratory outreach to the acute medical unit there.
This post therefore represents an opportunity to deliver excellent care to patients with respiratory disease, bridge the gap between primary and secondary respiratory care, help flow through the hospital by advising the Virtual COPD ward and help provide clinical leadership in COPD care. The post would be suitable for a post-CCT candidate looking to develop as a consultant in a supportive team in a large organisation.
Main duties of the job
The Imperial Community respiratory service is a consultant led service overseeing the overall management of patients with a chronic lung condition, from diagnosis to end of life, living in the inner boroughs of Westminster, Kensington and Chelsea and Hammersmith and Fulham.
You will join a team of 30 health care professionals and 6 consultants who provide;
-Outpatient clinics within community sites for diagnosis and optimisation of respiratory disease at St Mary’s St Charles’ in Ladbroke grove, Maida vale clinic, Violet Melcher and others.
-Early supportive discharge.
-Admission avoidance.
-Disease optimisation.
-Pulmonary rehabilitation.
-Home oxygen services.
-MDT working with Diagnostic hubs, breathlessness services and COPD optimization.
-Education and support across primary and community care.
The service works seamlessly with colleagues within Imperial College Healthcare NHS hospital sites and the NWL remote monitoring hub.
Person Specification
Qualifications
Essential
- GMC registered Medical Practitioner
- MRCP(UK) or an equivalent qualification
- Entry on the General Medical Council (GMC) Specialist Register via one of the following: a.Certificate of Completion of Training (CCT) (the proposed CCT date must be within 6 months of the interview) b.Certificate of Eligibility for Specialist Registration (CESR) c.European Community Rights
Knowledge/ Skills
Essential
- CCT in Respiratory and general internal medicine
- Experience of MDT working
- Experience of virtual wards
- Experience of community COPD MDT
- IT Skills and computer literacy
- Knowledge of airways disease management
- Competency in NIV/ management of acute respiratory failure
- Experience of leading acute unselected medical take
