Job summary
Our Rapid Assessment and Prevention OT service have an opportunity for a static Band 6 Occupational Therapist to join the team.
RAP works collaboratively with GP and Welsh Ambulance colleagues to intervene at the earliest point of crisis with a joint aim to support individual’s to avoid the need for conveyance to Accident & Emergency.
The operating model has one OT based within the Clinical Navigation Hub at Ty Elai, with two OTs ‘roaming’ providing patient facing clinical responses. OTs will rotate between being Hub based and roaming, utilising hot desks across the CTMUHB and Local Authority sites to reduce unnecessary travel and network with our in-patient and community colleagues. Primary base for this post will be Clinical Navigation Hub at Ty Elai.
We need a dynamic practitioner with a passion for community based working, with a clear focus on admission avoidance and rapid responses to keep individuals in their own homes.

Main duties of the job
The primary focus will be rapid multi-factorial assessments in individuals homes to identify what matters to them, to co-produce achievable goals and implement fast pace interventions.
The post-holder will work collaboratively with our integrated Health and Local Authority community teams delivering on place based care ethos.
You will prioritise and screen referrals with the aim to offer a same day response. This will include telephone consultation, home based assessment, with scope to complete an additional 2 sessions to support individuals to return home during their acute need. Your scope of practice will include compensatory and functional rehabilitation methods to prevent admission and support the person to manage their immediate needs. You will form close working relationships with your integrated primary care and rehabilitation colleagues to handover the episode of care to support the individual to work towards achieving longer term goals.
You will be part of the service design and will work closely with your Band 7 Clinical Lead, Band 6 OT colleague and Principal Occupational Therapist to implement change at pace.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications
Essential
- Degree/Diploma in Occupational Therapy
- Post graduate training/accreditation in fieldwork education
- Knowledge and application of principles of clinical governance and risk management
- Detailed knowledge and understanding of professional ethics/standards and code of conduct, and their application in practice
- Evidence of critical appraisal/analytical thought.
- Knowledge of graded therapeutic interventions.
- Knowledge and understanding of factors which can influence discharge planning including: cognitive, physical, psychological and cultural
- Knowledge of broad range of medical, surgical and orthopaedic conditions
Desirable
- Knowledge of national drivers for health provision for service user group
- Management qualification
- Post-graduate training in motivational interviewing, goal planning
- Interest and involvement in quality, audit and service evaluation and non-standardised assessments that can be used with service user group
- Understanding of the theory and practical application goal orientated approach

- Post graduate clinical experience in area of physical medicine
- Experience of service delivery in a variety of different clinical physical health settings
