Job summary
The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for managing their cancer journey. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
The post holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive i.e. co-ordinating health and wellbeing clinics to enable independence and support self-management as appropriate.
The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.
The post holder will also perform basic tasks where necessary including taking patients observations and assisting with patient needs, preparing them for treatment and also assisting with their personal needs whilst treatment is ongoing.
Main duties of the job
Provide regular supported conversations with the person living with cancer,
Following a holistic needs assessment, co-ordinate care and development of a personalised care plan
Provides practical and emotional support to people living with cancer and their families.
Triage individuals contacting the service face to face or by telephone
Work unsupervised,
Respond to any queries from patients, carers and visitors
Implement, monitor and review the care plan with the patient and carer
Co-ordinate the care for a defined group of patients
Co-ordinate the necessary assessments, appointments or investigations
Co-ordinate remote surveillance and monitoring through appointments
Act as advocate and facilitator to resolve issues that may be perceived as barriers to care
Document and monitor all aspects of care co-ordination and service delivery,
Provide a wide range of administrative and clerical support
Support the MDT process
Assist the team to complete audits
Create and maintain databases which record all support and information activity
Liaise closely with the MDT co-ordinator ensuring robust systems of notification
