We are pleased to announce an exciting opportunity for internal staff to join Our Erewash Older Adult Community Mental Health Team (CMHT) and Living Well Team in Ilkeston and excited to welcome a Band 6 Older Adult Specialist to our supportive and welcoming team.
Main duties of the job
About the Role
We’re looking for someone with experience working with older adults in the community who are living with functional mental health conditions. This brand-new role, based within the Older Adults CMHT, offers an exciting opportunity to shape meaningful support for individuals in our local area. You’ll be playing a key part in introducing people into the Living Well Team, helping to identify their needs and designing person-centred, compassionate support that empowers them to live their best life. Collaboration is at the heart of this role. You’ll be working alongside colleagues across the community to ensure that the right support is delivered at the right time by the right person.
About the Teams
While the role is supported by the Older Adults CMHT, you’ll also be working closely with the Living Well Team, a multi-agency group that places individuals right at the centre of their care. One of the most rewarding parts of this role is being part of a team that reaches out to people who may not have had access to mental health support before. The successful candidate will need to have the ability to travel across the geographical area.

Qualifications
Essential
- Relevant qualification for the post (Diploma in Nursing/Degree in Mental Health Nursing) Degree in Occupational Therapy
Desirable
- Assessor qualification
- Undertaken further training relevant to the role
Experience
Essential
- Post qualification experience in the relevant field / Compteted preceptorship or equivilent
- Applicant has experience in working with Older people with functional mental health diagnoses
- Experience of community working
- Able to meet the travel requirements of the post
- Good verbal/ non verbal communication skills
- Good IT skills
- Able to work independently, as well as part of a team
Desirable
- Experience of linking with partner agencies
- Understanding of the living well programme
- Good time management
