Senior Children’s Palliative Care Team Nurse

Job summary

An exciting opportunity has arisen within the Manchester Local Care Organisation which is part of the Manchester University NHS Foundation trust for a Band 6 to expand there knowledge and skills within Children’s community Palliative care.

We are seeking a Registered Children’s Nurse band 6 to join our small team.

We currently have 1 post available 37.5hrs per week working a four day pattern.

The individual would support families and patients in there own home and in the hospital setting. Acting as patient advocate and coordinating care. Clinical skills are important, along with active listening skills and good communication.

Main duties of the job

We are looking for a highly motivated and enthusiastic nurse with excellent communication skills and good organisational skills and leadership skills, Who can operate as an autonomous practitioner, delivering evidence, based nursing care to children and their families. You will support the team leader in managing team members along with innovations to continue to develop the team.

The Children’s Palliative care team supports children in the community with life limiting and life threatening conditions. Although this is a seven day service you would be expected to operate over five days, from 8am-5pm. You will be part of a 24 hour on call rota, when it is necessary for a child to receive end of life care at home.

You will be part of a forward thinking and friendly team, providing holistic care to families within the community. You will be expected to work closely with our short break facilitators and other professionals within the hospital, community and wider team.

Essential

• Registered Nurse RSCN/RN Child
• Study to Degree level or prepared to work towards
• Teaching and assessing certificate
• Evidence and experience of working with complex patients in different situations
• Effective communication skills (listening, speaking and advocating)
• Evidence to support professional leadership
• Understand the holistic approach to patient care and how this delivered in the setting
• Evidence to support your knowledge regarding palliative care, end of life and bereavement support
• Demonstrates motivation and commitment towards personal and professional development.
• Able to work independently and in partnership with members of the multi-disciplinary care team.
• Able to prioritise work and effectively manage time of self and others.
• Able to work unsupervised and in autonomous manner.

• Ability to work flexible shift patterns including 24 hour on call

Desirable

• 1st degree/specialist practitioner’s degree CCN.
• Palliative care or advanced masters practice modules.
• Experience of managing a complex caseload, where competing demands and constant interruptions to the work pattern require workload to be prioritised.
• Experience of working with children and young people with complex medical needs, learning disabilities and complex behaviours.
• Ability to organise, coordinate and chair multidisciplinary meetings
• Experience of managing a team, training others and ability to discuss change in a positive way
