An exciting opportunity has arisen for an enthusiastic Band 4 Assistant Practitioner to join our dedicated team. A great opportunity to develop your rehabilitation skills and knowledge.

This varied role will involve working in the community and the Rehabilitation Centre at Wigton Hospital

You will be helping helping individuals to achieve goals that are important to them, supporting hospital discharges, prevent avoidable admissions and improve the health and wellbeing of our Neighbourhood. This will include 1:1 and group work including non complex assessment and outcome measures. Interventions include Exercise programmes equipment and adaptation prescription, and practicing daily living skills.

Training is provided and we are committed to team and individuals development

The candidate would need to either hold the band 4 Assistant Practitioner qualification or equivalent, or demonstrate that they would be able to meet the requirements to undertake the Assistant Practitioner qualification at the next enrolment opportunity.

Although based at Wigton Hospital there is a requirement to cover the Keswick & Solway footprint if required

Main duties of the job

  • Manage a community therapy caseload, completing rehabilitation treatment plans and treatment sessions.
  • Working closely with Occupational Therapists, Physiotherapists, District Nurses and wider ICC team members on joint treatments and visits.
  • Supporting admission prevention through rapid response visits.
  • Following the D2A model in both community hospital and acute settings to support discharges.
  • Completing a variety of assessments including but not limited to; personal care, kitchen assessments, mobility assessments, falls and environmental assessments.
  • Assess and provide equipment and adaptations
  • Work with patients to maximise their rehabilitation potential which includes providing an actioning therapeutic activities.

Person Specification

Qualifications

Essential

  • Have a Foundation Degree – Health and Social care, (Assistant Practitioner route) or equivalent.
  • Maintenance of professional portfolio. Level 2 Maths, Level 2 English or equivalent, Care Certificate

Knowledge and Experience

Essential

  • Significant experience of working within a healthcare environment.
  • Experience of caring directly for patients, relatives and carers
  • Experience working with in a multidisciplinary team
  • A basic understanding of the use of IT
  • Understanding of and adherence to boundaries of practice and professional conduct

Desirable

  • Experience of working in a community rehabilitation setting
  • Experience of working in health promotion
  • Experience of using outcome measures
  • Knowledge of community working
  • Knowledge of exercise and activity recommendations for healthy lifestyles
