Job summary
The Specialty Doctor will play a key role in delivering high-quality medical care to patients on Anstey Ward at Alton Community Hospital, part of Hampshire and Isle of Wight Healthcare NHS Foundation Trust. Working within a multidisciplinary team that includes advanced practitioners, consultant nurses, general practitioners and therapists, the post holder will provide comprehensive clinical assessment, treatment, rehabilitation and ongoing management of patients requiring inpatient community care.
The role supports a range of services including rehabilitation, reablement, discharge-to-assess pathways, step-up and step-down care, and end-of-life care. The successful candidate will work closely with consultant geriatricians and partner organisations to ensure safe, effective and person-centred care that promotes recovery, independence and positive patient outcomes.
In addition to direct clinical responsibilities, the Specialty Doctor will contribute to the clinical leadership of the service, supporting the development of care pathways, quality improvement initiatives, governance activities, audit, teaching and service development.

Main duties of the job
Ensuring evidence-based practice and service user-centred recovery principles to assess, plan, implement, monitor and evaluate all interventions provided by the liaison team to promote recovery and independence.
Managing complex information and make formulations using a biopsychosocial model.
Ensuring appropriate and effective engagement with carers and ward teams.
Management of risk and weighing complex risk information to deliver care that is in the best interests of the recovery of the individual. Supporting the MDT in this by clear communication, containing anxiety, and maintaining hope for the patient.
Ensuring the service works in line with care planning, risk management and safeguarding policies.
Maintaining a focus on outcomes, both in terms of clinical outcomes and patient experience.
Supporting learning from good practice, incidents, complaints or concerns across Hampshire Hospitals and the liaison team.
To escalate any shortfalls in clinical practice in a timely fashion to the Divisional Medical Director

Qualifications
Essential
- Full Registration with the General Medical Council
- Considerable experience in general medicine and geriatric medicine
