Job summary
The post holders will be an integral member of the Enfield Adult Community Rehab Team. 1 role is 37.5 hrs, 2nd role is 18hrs 45mins.
The post holders will be developed to take short term responsibility for the effective planning and delivery of care to patients in co-operation with other team members and in communication with other clinical team leaders.
The post holder will be required to support clinicians staff with oversing education programmes and also to visit patients in their own homes to carry out prescribed treatment and assess for and provide appropriate O/T equipment or mobility aids directly or by prescription.
The post holder will work alone and will be required to use a degree of initiative while working within the parameters of the team’s policies and procedures.
The post holder may be required to work at times in other services dependent on integration and service need.
The post holder will be an integral member of the multi professional teams, which is in partnership with the voluntary sector in delivering and their carers.
A full UK driving licence Car owner/driver for work purposes.
Main duties of the job
To undertake clinical work delegated from the Occupational Therapists and Physiotherapists.
To be responsible for a designated caseload of patients.
To work effectively as a member of a team in providing a programme of community based therapeutic activities.
To contribute to all stages of the therapeutic process including: – Referral taking and information gathering through liaison with clients, their informal carers, and colleagues from health and other agencies.
Completing simple assessments, through interview and observation of functional tasks.
Identifying needs and forming goals and plans for interventions.
Completing a wide range of interventions including: – giving advice; home exercise programmes; rehabilitations programmes; equipment provision; and simple home adaptations, such as, banister rails and half steps.
Evaluate client progress through regular re-assessment and ensure continuous progression by graded activity.
Contribute to formal therapy assessments and provide feedback.
Review of clients after completion of minor adaptations to home environment.
Discharge planning and discharge.
Person Specification
Education and Professional Qualifications
Essential
- Relevant qualification e.g. C & G Health & Social Care NVQ 3 or equivalent
- Training courses relevant to care group
- English qualification at GSCE or equivalent level.
Desirable
- Trusted Assessor
Experience
Essential
- Experience at working at band 3 level in a physical rehabilitation setting or experience within a social care setting
- Knowledge of the ranges of equipment relevant to clinical area
- Knowledge and experience of working with patients who have physical health disabilities / or impairments
- Evidence of working with and caring for adults either in an acute, community NHS or social care setting.
