Job summary
We are seeking to appoint a substantive Consultant in the Community, Frailty & Therapy Care Group to join a team of three existing Consultants and 3 Specialists on the south bank.
The successful applicant will have the opportunity to be appointed as Honorary Senior Lecturer at Hull York Medical School (HYMS). This honorary appointment is in recognition of contribution to teaching and research and holders of honorary titles may request access to HYMS and University premises and facilities, systems and library services.

Main duties of the job
We are seeking a passionate and dedicated Geriatrician to join our multidisciplinary team in providing exceptional care for older adults . As a key member of our team, you will support patients living with frailty both on the acute hospital sites but also working closely with our wider system and community colleagues to ensure our frial patients are cared for in the right place, at the right time and by the most appropriate team
Key Responsibilities:

Clinical : Provide expert clinical assessment and decision-making for frail older adults at the “front door” of acute care (e.g., Emergency Department, Same Day Emergency Care, Frailty wards).

Comprehensive Geriatric Assessment (CGA): Lead or support the delivery of CGAs to ensure rapid and holistic assessment of frailty, medical needs, functional status, cognition, and social factors.

Multidisciplinary Collaboration: Work closely with nursing staff, allied health professionals, geriatricians, social workers, and community services to enable timely, person-centred care.

Admission Avoidance: Develop and implement plans to avoid unnecessary hospital admissions, facilitating safe discharge or transfer to appropriate community-based services.

Pathway Development: Contribute to the development and improvement of frailty pathways, including rapid access clinics, discharge-to-assess models, and integrated care solutions

Qualifications
Essential
- MBBS
- Equivalent to MBBS
- Entry on the Specialist Register via CCT (proposed CCT date must be within six months of interview), CESR or European Community Rights
Experience
Essential
- Undertaken programme of training acceptable to the Royal College of Physicians for appointment to Consultant Status
- Entry on the Specialist Register via CCT (proposed CCT date must be within six months of interview), CESR or European Community Rights
- Able to take responsibility for delivering the service without direct supervision
- Applicant’s knowledge is up to date and fit to practise safely
- Appropriate to the holding of a Consultant in Geriatrics and General(Internal) Medicine
