We have an exciting opportunity for a Band 5 enthusiastic, motivated, and keen nurse to join our established multidisciplinary Care Community Team at Chester South to support and deliver a range of quality nursing services to our patients.

Main duties of the job

Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. It’s important that you can drive and have access to your own vehicle for work. But, of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

Essential

  • Registered General Nurse
  • Evidence of recent training

  • Evidence of working towards or Degree in Professional Practice
  • Student Mentorship

  • Awareness and understanding of the Public Health Agenda
  • Evidence of research based/reflective practice

  • Demonstrate highest standards and skill in nursing.
