Job summary

The States of Guernsey are looking for a Senior Staff Nurse to join the Rapid Response and Reablement Team within Community Health and Social Care Services.

Main duties of the job

Adult Community Health and Social Care Services is a multidisciplinary team
containing Community Nurses, Social Workers, Social Care and support staff.

As a Senior Staff
Nurse in the Rapid Response and Reablement Service, the post holder will work alongside the
Team Lead and therapists in the management of the team and a defined caseload to provide
high quality patient care.

To provide assessment, planning, implementation and evaluation of
care through multidisciplinary work, and to assist the Team Lead to undertake performance
reviews and regular supervision of the team members.

Person Specification

Qualifications

Essential

  • Registered Nurse with substantive experience at Band 5 level in a community setting
  • Current registration with NMC
  • Diploma in relevant field

Desirable

  • Degree in relevant field in Health and Social Care.
  • Approved mentor preparation course or equivalent
