Working Full time : 37.5 hours per week
An exciting opportunity has arisen at Countess of Chester Hospital for an Advanced Clinical Practitioner (ACP) to be part of the Frailty team who manage the SDEC Frailty Unit at Countess of Chester Hospital.
The successful ACP will work autonomously providing, clinical care, expertise advice and leadership within the Frailty Unit.
The successful applicant will also ensure that high quality patient care is maintained by autonomously making / maintaining clinical decisions and treatment plans to manage acute illnesses, whilst engaging within the excellent ethos of multidisciplinary working to ensure effective and efficient patient care.

Qualifications
Essential
- RN Diploma/Degree or Paramedic qualification
- MSc Advanced Practice/Clinical Masters
- Non-Medical Prescriber (V300 Extended Formulary and Supplementary Nurse Prescribing Programme).
- Relevant clinical skills to support clinical practice.
Desirable
- Post graduate Certificate in Education
- Trained in Advanced Life Support (Adult and Paediatric)
Knowledge and Experience
Essential
- Knowledge of advanced clinical, psychological, and social needs of patient care
Desirable
- Leadership experience
