Are you an ambitious Occupational Therapist ready to make a real difference? Join Humber Teaching NHS Foundation Trust as a Specialist Integrated Community Occupational Therapist within our Intermediate Care team serving Ryedale, Scarborough and Whitby.

Working in a pioneering partnership with North Yorkshire Council, you’ll deliver rehabilitation and reablement across community and bed-based settings, helping people with complex physical, functional and cognitive needs maximise independence, avoid unnecessary hospital admissions, and achieve safe, timely discharge.

As part of a skilled multidisciplinary team, you’ll work autonomously, use advanced clinical reasoning, provide evidence-based interventions, and contribute to service improvement. You’ll also support colleagues, champion integrated working, and help drive a true “One Team” approach.

Main duties of the job

• Deliver specialist occupational therapy assessments and interventions across community and bed-based Intermediate Care services.
• Support people to regain independence, improve function, and achieve personalised rehabilitation goals.
• Manage a complex caseload using advanced clinical reasoning and professional judgement.
• Work collaboratively within an integrated multidisciplinary team to prevent hospital admissions and support hospital discharge.
• Provide clinical leadership, supervision, training and support to junior staff, assistants and Generic Support Workers.
• Assess, prescribe and review equipment and adaptations to enhance safety and independence.
• Contribute to service development, quality improvement, audit activity and evidence-based practice across the integrated care pathway.

Person Specification

Qualifications Education and Training

Essential

• Current HCPC professional registration
• Professional knowledge acquired through degree or equivalent, plus short specialist courses, experience to post-graduate equivalent diploma level
• Specialist understanding of relevant clinical practice, standards and protocols within identified clinical area

Desirable

• Previous experience working as a community based Occupational Therapist
• Member of specialist interest group/network, locally or regionally
• Leadership qualification/equivalent experience if required

Knowledge and Experience

Essential

• Evidence of sound post-registration professional practice
• Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care
• Experience of case load management within specialist service area, some of which may be complex

Desirable

• Working knowledge of Trust policies and procedures

Skills, Competencies and Personal Qualities

Essential

• Effective inter-personal skills and experience in supervising and fieldwork education/mentorship
• Ability to effectively commute between the organisations various sites and patient homes
• Required to work within North Yorkshire Council while receiving supervision and support from HTFT.

Desirable

• Ability to manage daily operations and maintain a safe working environment
