We are looking for an enthusiastic and motivated Lead Pharmacist to join our team and provide clinical leadership across our anticoagulation services.

This is an exciting opportunity for a pharmacist who is passionate about improving patient outcomes, optimising medicines use and developing high-quality services.

You will play a key role in leading and developing the service, ensuring the delivery of safe, evidence-based and person-centred care to patients requiring anticoagulation.

You will work collaboratively with multidisciplinary teams, healthcare professionals and patients to optimise anticoagulant therapy, promote best practice and drive continuous service improvement.

Main duties of the job

Provide clinical and professional leadership for anticoagulation services.

Support the safe and effective use of anticoagulants.

Lead the development, implementation and evaluation of anticoagulation pathways and service improvements.

Provide expert advice and education to healthcare professionals, patients and carers.

Promote medicines optimisation, patient safety and adherence to relevant guidelines and policies.

Work collaboratively with medical, nursing, pharmacy and wider multidisciplinary teams.

Support audit, quality improvement and clinical governance initiatives.

Contribute to the education, training and development of pharmacy colleagues and other healthcare professionals.

Person Specification

Experience

Essential

  • Significant experience in all aspects of hospital pharmacy
  • Training and/or significant experience in clinical teaching/tutoring, audit, practice research, risk management.

Desirable

  • Experience of working in a similar role

  • Accurate and concise written and oral communication skills.
  • Able to receive, summarise, communicate and present information to wide range of professionals across the Trust.
  • Proven persuasion and negotiation skills (to influence prescribing), tact and diplomacy skills.
  • Effective independent and team working skills.
  • Ability to analyse and interpret a wide range of clinical information.
  • Proven problem solving approach on a wide range of clinical issues, and able to think laterally to develop new ideas.
  • Able to assess staff performance against agreed standards and criteria, and to provide feedback and development.
  • Able to, with consultation, prioritise, plan and adjust current pharmacy services within the specialised area.
  • Able to work effectively to short, medium, long term and changing deadlines.
  • Able to work effectively in front of an audience.
  • Able to review and provide specialised advice on medicine use in specific clinical areas.
  • Able to obtain, interpret, analyse and report on drug expenditure within the specialised area.

  • Computer literate.

Knowledge

Essential

  • Understanding of Information Governance and Confidentiality.
  • Broad knowledge of therapeutics.
  • Specialised knowledge of therapeutics and medicine use within acute medicine.
  • Comprehensive knowledge and understanding of the formulation, preparation, and therapeutic action of medicines.

Qualifications

Essential

  • MPharm or equivalent.
  • Registered as a Pharmacist with the General Pharmaceutical Council (GPhC)
  • Diploma in Clinical Pharmacy or equivalent experience.

Desirable

  • Member of the Royal Pharmaceutical Society of Great Britain.
  • Independent Prescribing qualification.
  • Leadership / Management qualification.
  • Working towards Faculty Accreditation.

Other Requirements

Essential

  • Demonstrates continued professional development or equivalent knowledge through extensive experience.
  • Flexibility in working hours to suit the needs of the department.

Desirable

  • Current driving licence/able to travel between sites.
