Job Summary
An exciting opportunity has arisen for a highly motivated, Health Support Worker to join our team. The successful candidate will have the opportunity to work as an integral member of a multidisciplinary team which provides expert, caring and holistic supportto adults with a diagnosed learning disability.
You will be supported to develop skills to carry out a variety of interventions delegated by nursing or allied health professionals (Speech and Language, Occupational Therapy, Dietetics and Physiotherapy). You will work with people who have a learning disability and physical health care needs such as issues relating to communication, mobility, nutrition and dysphagia.
You will be required to work in a variety of community settings, with individuals, groups, families and staff teams. You will have regular supervision and will be expected to support the multidisciplinaryteam as a whole. Duties may include direct intervention but also developing resources and supporting training.

Main duties of the job
- To support nursing and allied health professionals (Speech and Language, Occupational Therapy, Dietetics and Physiotherapy) within the multi-disciplinary team to deliver safe and person-centred care.
- Work with service users to provide interventions through ongoing support, practical assistance and problem resolution under the supervision of a senior clinician.
- To contribute to the regular assessment, and reviews of services users’needs, development of care plans and provision of appropriate interventions.
- To work in partnership with other agencies, teams, and carers to meet the needs of services users.
