Job Summary
The MYTT Specialist Palliative Care Team have an exciting opportunity for a highly motivated individual with a passion for palliative care, who has the correct skill set to join the team as they continue to go through their transition of change.
Main duties of the job
The post holder will support and assist the Palliative care team with the day-to-day management of patients that are referred to the SPCT. The post holder will support the CNS to triage referrals, booking patients in to the CNS diary, making direct contact with the patient and signposting where appropriate. The post holder will support and coordinate the running of outpatient clinics, booking clinic appointments and typing up assessments from these clinics. The post holder will enable and assist the CNS team in delivering high standards of care to all patients that are referred to the SPCT who have a palliative care diagnosis and may potentially be in their last year of life.
