Job Summary
An exciting opportunity has arisen to join the Swansea Community Integrated Therapies Service in the Swansea Bay University Health Board. This involves working in a variety of Community based settings across Swansea Bay.
The post will provide an opportunity, as part of a multidisciplinary team, to support therapy input and provide rehabilitation and discharge planning to patients and groups of patients in a number of settings, including patients’ own homes and care homes, reablement bedded facilities and Community Exercise Classes.
The post holder will need to be able to adopt a flexible approach to work and will also be expected to participate in 7 day working rotas.
The post holder will need to be able to demonstrate relevant experience of working in rehabilitation in the NHS and or Social Care or Community based settings. Previous Therapy Assistant experience is highly desirable for this role.
An existing NVQ 3 /AGORED qualification/ evidence that you are working towards the Level 3 Diploma in Rehabilitation or other relevant healthcare or exercise diploma is essential.
The ability to travel independently between sites across the service is also essential so the successful candidate must be able to do this in a timely manner.
The role will also entail some administrative responsibilities, so good basic IT knowledge and/or understanding of healthcare IT systems is highly desirable.
You will be supported by qualified therapists and the clinical team leads.
Main duties of the job
As an Assistant Practitioner (Level 4) the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. They must have an appropriate understanding of evidence based practice to deliver care in line with current evidence, the post holder will take responsibility for taking action relative to an individual’s health and care in accordance with organisational policy and procedures. The post holder will be responsible for some elements of assessment, implementing programmes of care and modifying individualised care plans, reporting back to the registered practitioner. They may delegate work to others and may supervise, teach and assess other staff. In Swansea CRT the Assistant Practitioner provides intervention to West Ward, Gorseinon, Bonymaen House, hospital discharge patients requiring reablement and community dwelling individuals in their own homes or care homes.
