The Role:

We are seeking a compassionate and experienced Care Coordinator to join our dynamic team. This vital role involves managing the day-to-day operations of our Home Care services, ensuring the delivery of high-quality care and support to our clients.

Key Responsibilities:

  • Develop, implement, and review individualized care plans in partnership with clients and their families.
  • Coordinate and supervise a team of care workers, ensuring all staff are trained to the highest standards.
  • Maintain accurate records and reports in compliance with CQC regulations and company policies.
  • Audits to high standards, with action plans.
  • Ensure that all aspects of care and service comply with regulatory standards.
  • To supervise staff and ensure that all staff contribute to the efficient running of the service
  • Oncall duty at least 1 -2 week in a month, in rotational
  • This list is not exhaustive and additional duties may be required.

Requirements:

  • Proven experience in a senior role within a Domiciliary Care setting.
  • NVQ Level 3 in Health and Social Care or equivalent is essential.
  • Excellent organizational and leadership skills.
  • Ability to work flexibly to meet the needs of the service.
  • Strong communication and interpersonal skills.
  • Commitment to upholding the rights and welfare of residents.
