Job Summary

The North Manchester Early Intervention for Psychosis Team is delighted to be able to offer a full-time permanent post of care coordinator: registered nurses, social workers or occupational therapist are welcomed to apply.

The Manchester Early Intervention for Psychosis Service is divided into three teams covering the city: north, central and south. This post will be based in the NORTH team – Central Park, Northampton Road, however you will be expected to travel across the city as part of the back-up assessment practitioner rota and across the Greater Manchester to inpatient units as part of the role.

You will work alongside a team of passionate and experienced colleagues and will be supported by the Clinical Team Manager and Senior Practitioner.

This post is a full time position (37.5 hours per week). The Team is currently operational Monday to Friday 9am to 5pm but flexible working can be discussed.

 

Main duties of the job

  • Overseeing the clinical management of a case load of service users age 14-65.
  • Complete social care act assessments and attendance at Funding Panel.
  • participation in duty rota – service wide triage worker,back up assessment practitioner for service wide assessments, back up worker for North team.
  • To encourage and support clients to collaborate in their treatment programme and recovery.
  • To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.
  • To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
  • Promote a culture of learning, education and professional development – the team often has student nurses/social workers on placement.
  • Adherer to the principles of safeguarding of children and vulnerable adults in line with Trust Policy and local reporting procedure.
  • Participate in safeguarding s42 investigations.
  • To undertake specialist mental health assessments when required in line with the Mental Capacity Act 2007, The Mental Health Act and Local Authority Safeguarding Adults Procedures.
  • To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm or risk of physical, sexual or emotional harm to others, using the Risk Assessment Tool.

Qualifications/ Education

Essential

  • Core professional Qualification i.e nursing/social worker/occupational therapist

Desirable

  • Evidence of continued professional development

Experience

Essential

  • Experience of supervising students in relevant field
  • Knowledge of requirements under The Care Programme Approach and Risk Management
  • Understand the importance and need to safeguard children and vulnerable adults.
  • Ability to engage with supervision – clinical and managerial.

Desirable

  • Ability to conduct first line assessments for individuals newly referred into the service, utilising a biopsychosocial framework
  • Experience of working with children and adolescents with a mental illness
  • Experience of working with individuals with Psychosis
