Summary

We are seeking a Care Coordinator to join our team in the GB area. Based in Reading, this role is vital in coordinating personalized care plans and ensuring our clients receive exceptional support aligned with our values of compassion and quality.
Responsibilities

  • Coordinate and oversee individual care plans to meet client needs.
  • Liaise with clients, families, and care staff to ensure smooth communication.
  • Schedule and manage caregiver assignments efficiently.
  • Monitor service delivery to maintain high standards of quality.
  • Maintain accurate documentation and compliance with regulatory requirements.
  • Support the recruitment and onboarding of new care staff.
  • Assist in developing new care strategies to improve service delivery.

Qualifications

  • Proven experience with office administration or coordination roles.
  • Strong organizational and communication skills.
  • Ability to manage multiple priorities effectively.
  • Compassionate approach aligned with person-centred care principles.
  • Knowledge of health and social care regulations is a plus.
  • Preferred: Experience working within a home care or healthcare environment.
  • Excellent interpersonal skills and attention to detail.
