This Job Description and Person Specification is a statement of the core duties of a Care Co-ordinator employed by My Versatile healthcare Ltd
Accountability
The Care Co-ordinator is accountable to, and reports to the Registered Manager and/or Director of My Versatile Healthcare and to the individual receiving the support.
Role and Responsibility
- The main responsibility of the Care Co-ordinator is ensuring the highest level of care and support is provided to our service users and carers.
- You will be responsible for efficiently organising and managing the scheduling of care staff, coordinating individual needs, and maintaining effective communication channels.
- Your role is critical in enhancing the quality of life for our individual while fostering a collaborative and compassionate work environment for our care staff.
- The role requires the ability to use your own initiative and work independently and creatively, communicating well with individuals and staff to ensure care calls are allocated accordingly and updating the team with any changes to individuals’ schedule.
- The Care Co-ordinator will be expected to fully participate in the on-call system operated by My Versatile Healthcare and complete on-call logs as necessary.
Main Responsibilities
To ensure the individual receives the highest quality of support in line with the values of My Versatile Healthcare Ltd Care Co-ordinator will be employed:
- To develop and maintain care staff schedules to ensure timely and appropriate coverage for all individual needs.
- To match care staff to individuals based on skill sets, availability and individual preferences.
- Address scheduling conflicts or emergencies promptly to minimise the disruption to service user’s care and support.
- Conduct initial assessments of new individuals to understand their individual needs, preferences and care goals.
- Collaborate with individuals, their families and healthcare professionals to develop personalised care plans.
- Regularly review and update care plans based on individual evolving requirements.
- Maintain open and effective communication with individuals, care staff, families and health care professionals/stakeholders
- Provide regular updates on individual condition, care plan and any changes to the care team.
- Document all relevant information accurately and confidentially in the care management system.
- Address individual and care staff concerns professionally and promptly, aiming for swift resolution.
- Proactively identify potential issues and implement preventative measures to ensure smooth care delivery.
- Escalate complex situations to management when necessary, offering recommended solutions.
- Assist with on boarding and training new care staff, ensuring they understand individual care plans and expectations.
- Provide ongoing support and guidance to care staff, promoting a positive and collaborative team environment.
- Conduct regular performance evaluations and provide constructive feedback to care staff.
- Maintain accurate records of individual information, schedules and communication.
- Assist in the preparation of reports, invoices, and other administrative tasks as required.
- The person must have experience and wants a long term involvement in growing the business as this is a fairly small business and aspiring to grow.
- The person must be willing to cover calls/visits and office work whilst recruiting staff and growing the business. There is opportunity to progress to management and salary increment once this has been achieved and as the business progresses.
- Answering calls and making calls to initial referrals who are approaching us for care
- Arranging for face-to-face initial assessments with clients and their families
- Completing and reviewing care plans and risk assessments as required
- Carrying out audits and ensuring CQC compliance is up to standard
- Delivering a service that meets regulated legal and ethical standards, ensuring services delivered meets regulated legal and ethical standards
- Ensure calls are covered and will also be expected to cover calls in emergency situations.
- Scheduling of carer rotas, ensuring visits are efficiently coordinated taking customer preferences, locations and staff availability into account.
- Ensuring that holiday, sickness and emergency calls are efficiently covered, and all affected parties communicated with.
- Keeping the internal IT systems updated with changes in carers on an ongoing basis

Key duties
- To lead and support the team in partnership with the Registered Manager to monitor the individual’s mental, emotional and physical health.
- To ensure the team provide direct support that meets the individual’s person-centred support plan in accordance with their needs and wishes.
- To ensure communication is effective with the individual, their family, friends, other senior staff and relevant health professionals.
- To work using initiative, in partnership with other senior staff and the Registered Manager to find proactive solutions and ideas which ensure positive and consistent support for the individual.
- To take responsibility for ensuring the physical, emotional and mental well-being of the individual is considered, liaising with other senior staff, the Registered Manager and health professionals, as required.
- To adhere to, and implement My Versatile healthcare policies and procedures, as well as external policies relevant to the organisation, in line with compliance requirements.
- To ensure any concerns regarding the individual, and/or any safeguarding concerns are reported to the Registered Manager and/or Director at the earliest opportunity.
- To work in partnership with the Registered Manager to devise, implement and review person-centred support plans, risk assessments and guidelines as required.
- To develop, maintain and review effective internal communication systems (i.e. diaries, handover records, communication books and on-call logs).
- To develop and maintain effective external communication systems for families, commissioners of services and health professionals (i.e. monitoring forms, behaviour logs/records and accident/incident forms).
- To problem solve with the team and develop and implement proactive person-centred risk management approaches as directed by the Registered Manager.
- To work in partnership with the Registered Manager to ensure the effective induction of new staff, including the introduction to individuals and work shadowing.
- To undertake staff Competency Observations and Spot Checks and devise and work with the Registered Manager to implement staff development plans to ensure best practice is shared.
- To facilitate regular staff team meetings in conjunction with the Registered Manager.
- To provide regular updates to the Registered Manager of changes in services and staffing.
- To take part in regular Supervision and attend team meetings and training as required.
- Duties may vary from time to time in line with the need of the service and as agreed with My Versatile Healthcare.

Qualifications (Essential)
- Level 2 and/or 3 qualification in Adult Social Care
- Willingness to complete Mandatory Training as part of the commitment towards own professional and personal development.
