Job Summary
This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND)Community Team in South London and Maudsley (SLaM). The team location will be based at Jeanette Wallace House, 1 Edridge Road, Croydon, London, CR0 1FE.
The FIND service operates Monday to Friday exclusive of bank holidays from 09:00-17:00. The FIND service provides flexible, community-based support to individuals who require care across multiple London boroughs.
The role focuses on delivering high-quality, person-centred care within people’s homes and local communities, ensuring that each individual receives timely, compassionate, and practical assistance tailored to their needs.This position involves regular travel across several London boroughs, including Lambeth, Southwark, Croydon, and Lewisham. You’ll be part of a responsive service that adapts to where support is needed most, so confidence in navigating different areas and working independently is essential.
The FIND Team will provide safe transition for inpatients discharged from the inpatient Forensic Services to the community providing specialist assessment, treatment and consultation to the Forensic Service in order to ensure a smooth, safe and clinically justified pathway to individuals with mental illness, offending history and learning disability with or without neurodevelopmental disorder.
Main duties of the job
The postholder will have oversight of a small caseload managed in partnership with the Multi-Disciplinary team (MDT). The MDT team is a team ofmulti professionals with specialist skills and experience with the client group.
This will include clinical assessment, treatment, planning,implementation and monitoring of outcomes. The post holder will carry out nursing procedures to a standard that ensures safe and effective care and complies with The Nursing and Midwifery Council’s Code of Professional Conduct. The post holder will clinically supervise and appraise junior staff and students.
There will be an expectation that the post holder will facilitate user involvement in all aspects of service delivery.The postholder will oversee and provide assessment, planning, implementation and evaluation to people on their caseload within a variety of settings, and ensure records associated with this process are maintained.
The post holder will carry out risk assessments for people on their caseload for example, work with the FIND Team Lead and MDT to manage these situations effectively. The role will also involve an opportunity to provide support, training and advice for junior team members and/or Trainee nursing students
