Job summary
This is an exciting opportunity to join the Gwynedd Learning Disability Community Health team at Betsi Cadwaladr University Health Board.
This is a Band 6 Community Learning Disability Nurse position within Gwynedd. The post holder will work as part of the Learning Disability team based at Gwynedd ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in patient services, as well as other statutory agencies and the voluntary sector.
The post holder will ensure they deliver evidence based, safe effective interventions and clinical care, following the nursing process.
Main duties of the job
The post holder will ensure that they follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards.
The post holder will have excellent communication skills and will be a visible and accessible member of the team, patients and their families can access for assistance, advice and support.
The post holder will be expected to communicate and receive highly complex and sensitive information and able to analyse and interpret information and act appropriately where there may be barriers to acceptance.
The post holder will have a good understanding of the CHC process and also be part of the development of pathways and Maturity Matrix.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications and/or Knowledge
Essential
- RNLD Qualification
- Evidence of a degree level Qualification or equivalent qualification in line with NMC registration.
- Plus evidence of post grad education and of ongoing CPD
Desirable
- Management qualification and /or management experience
- Mentor/supervisor/assessor
Experience
Essential
- Significant amount of experience within the clinical speciality or a relevant nursing role.
Desirable
- Experience of working in a multidisciplinary team setting.
- Experience/knowledge of senior nurse role
Aptitude and Abilities

- Ability to communicate and receive complex information
- The ability to diagnose a problem and take corrective action
