Job Summary
Assistant Practitioner/ Nurse Associate East South Somerset Somerset District Nurses: We are looking for Band 4 Assistant Practitioner/Nursing associates. You will be working as part of the Yeovil/Wincanton somerset District nursing team and have direct contact with patients, service users or clients in providing high quality and compassionate care for people with nursing need within the Community that are in their own homes.Our Band 4 Practitioners work at a level above that of healthcare support workers and have a more in-depth understanding about factors that influence health and ill-health for people.
Main duties of the job
To provide healthcare to patients as delegated by the Registered Nurse. This care will involve physical care and activities which promote health and wellbeing for patients. To work as part of a team, delivering health care that focus on the direct needs of the patient. To work autonomously within the clearly defined boundaries of their role and carry out specific clinical tasks and responsibilities as delegated by the registered nurse. To provide planned nursing care and treatment to patients in the community as assessed by the registered nurse. To undertake reassessment of Waterlow, MUST and falls assessments as delegated by the Team Leader or registered nurse.
