Job Summary

Gloucestershire Health & Care deliver community health services for adults & older people within multidisciplinary teams closely aligned with Primary Care Networks.

The Integrated Community Team consists of nursing, therapy and Reablement teams working closely alongside Adult Social Care colleagues to provide support to people within their own homes. We work closely with GPs, local communities, acute and community hospital(s) and other partners to enable people to live as independently and for as long as possible within their own home. We are a clinically led organisation where individuals are valued for their unique professional contribution to ensuring best patient care. Continuing Professional Development is a key priority and we actively promote and develop the broadening roles that Physiotherapists and AHPs are able to contribute.

As this is a community-based role you will be required to have a full Driving Licence valid for the UK and your own vehicle for business use.

Main duties of the job

You will need excellent communication skills and commitment to high quality patient care alongside flexibility and resilience to meet the challenges of community working.

As a B5 you will need:

  • Relevant experience of working as part of an integrated multi-disciplinary team
  • Excellent communication and IT skills.
  • Experience of working in people’s homes
  • Be highly motivated to provide evidence based practice.

Qualifications

Essential

  • Diploma/Degree in Physiotherapy
  • HCPC Registration

Desirable

  • CSP Registration

Experience

Essential

  • Relevant clinical experience for treating patients within a community MDT
  • Evidence of autonomous working and managing caseload
  • Evidence of clinical assessment and reasoning tools in practice

Desirable

  • Experience of working within the NHS
  • Experience in the field of rehabilitation
