Welcome to South Waveney Primary Care Home (PCH) team.
Are you a Registered Nurse (Adult) or 3rd year student due to qualify? Do you want to progress your nursing career with a supportive and encouraging team, providing nursing care to patients in their own homes, clinic or residential homes?
We are looking for a dedicated and motivated Registered Community Nurse to join our team. Following a structured induction and with ongoing support, including a thorough Preceptorship period if needed, you will work autonomously within a community setting, managing a varied caseload safely and effectively.
The ideal candidate will be able to demonstrate the relevant qualifications, clinical skills and professional registration required for this role, along with the confidence and sound judgement to make independent clinical decisions.

The PCH team provides community services 7 days per week 07:00- 20:30 hours
The core shift is 08:30- 16:30, other shifts included in the rota are 07:00- 15:00 and 12:00 -20:00 as well as weekends and bank holidays on a rotational basis.

Main duties of the job
As a community nurse you will be providing an excellent standard of person centered care to adults in their own homes, residential care or clinic settings This will include a mixture of nursing and reablement interventions to support patients to maintain their health or recover from ill health where possible. You will be supported by Senior nurses to gain the clinical skills required to deliver care in the community and you will eventually hold your own caseload of patients.
You will complete holistic assessments for patients referred to the service. Participate in delivery of nursing interventions which could include, but not restricted to:
Holistic assessments including environmental, moving and handling assessments, pressure area risk, nutritional needs.
Urinary catheterisation
Administration of medication via injection
Wound care and specialist leg ulcer assessment and care
Palliation and end of life care
Acute admission avoidance
Support patient self care using health coaching

Qualifications
Essential
- *Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult)
- *BSc in Nursing Practice or equivalent
Desirable
- *Mentorship/ preceptorship qualification/ ENB 998
- *Evidence of working towards a nursing degree
Experience
Essential
- *Evidence of continued professional development
- *Ability to work unsupervised
- *Ability to demonstrate clinical knowledge and competence within scope of clinical practice
- *Previous experience of working in health care
Desirable
- *Recent clinical experience in community nursing
- *Experience of multidisciplinary team working
