Job Summary

An exciting opportunity has arisen for a part time Stroke Reviewing officer to join our established Integrated Community Stroke Service to deliver six and twelve month post stroke reviews and support the therapy team to delivery rehabilitation into various community settings.

Our award winning Integrated Community Stroke Service delivers rehabilitation 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future.

Main duties of the job

The core component of the role will be to provide specialist assessment, signposting and support via a designated reviewing process at 6 and 12 months after stroke

This role will also involve working collaboratively within our Integrated Community Stroke Multidisciplinary Team of AHP’s, together you all make the difference. You will be expected to:

  • Demonstrate your passion and clinical ability to assess patients who present with significant physical health care needs.
  • Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.
  • Assess, analyse and support clients, working collaboratively within a multidisciplinary framework, providing compassionate, realistic and timely support to a diverse patient group and their families.
  • Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings

Person Specification

Qualifications

Essential

  • NVQ 3 in Health or Health and Social Care or equivalent
  • Foundation degree or equivalent eg accredited CPD units

Experience

Essential

  • Experience of multi-disciplinary work. This could include substantial voluntary work or work in a related field eg housing.
  • Experience managing own work in a team setting
  • Experience of working with Stroke patients
  • Experience of record keeping, report writing, correspondence

Desirable

  • Experience of working as Assistant Practitioner
  • Experience of working as Rehab Support Worker
  • Community Working (stroke)

Knowledge

Essential

  • Good understanding of community care objectives and practices
  • Understanding of role of community stroke and the social services and their services
  • Familiarity with the role of relevant agencies. (eg health and housing)
  • Understanding of need and risk in relation to community care
  • Understanding of the concepts of single assessments and care planning
  • Understanding of the roles and skills of the members of the MDT
  • Awareness of a range of minor daily living equipment and adaptations
  • Awareness of rehabilitation and promoting independence

Desirable

  • Ability to empower patients / service users and enable them to live as independently as possible
  • Ability to undertake standard and specialist assessments
  • Ability to work in partnership with patents / service users and carers
