Diabetes Family Psychosocial Support Worker: Sponsorship Available

Working as a key member of the children’s and young people’s Diabetes team, supporting and motivating children and young people along with their carers/families to improve and maintain their health and wellbeing. Working alongside the multidisciplinary team and escalating concerns as required.

Main duties of the job

Working within a multi-disciplinary team to deliver high quality family/patient centred care and evidence-based psychosocial interventions. Working collaboratively across a range of disciplines, developing knowledge and expertise to enable them to perform non non-complex, protocol limited, clinical tasks with minimal supervision.

Providing advice and support on parenting issues related to diabetes and offer guidance to families on a variety of matters including housing, school and benefits.

Working within the Women and child division, you will work within a team of paediatric nurses providing evidence based care for our local families. Working within the hospital, outpatient’s, visiting the families at home and supporting within schools if needed.

Person Specification

Qualifications/training & professional development

Essential

  • Successful completion of Foundation Degree equivalent to Diploma level
  • Evidence of continued professional development documented through Trust PDR/KSF/Supervision processes and personal portfolio
  • Commitment to undertake further training and desire to develop new skills

Desirable

  • Evidence of recent update in Safeguarding Children

Experience

Essential

  • Demonstrable experience of working with children, young people, families and wider support networks
  • Knowledge of a range of relevant clinical services

Desirable

  • Experience of working with children and young people with diabetes
  • Experience of working in a community role
  • Knowledge and skill in assessment, planning, implementation and evaluation of the care plan

Skills, abilities & knowledge

Essential

  • Ability to self-manage time and meet agreed deadlines
  • Ability to work effectively as a multi-disciplinary team
  • Understanding of confidentiality
  • Understanding of equality and diversity issues
  • Understanding of safeguarding children
  • Ability to travel independently e.g. full driving license

Desirable

  • Clinical skills e.g. measurement and documentation of height, weight, blood pressure, urinalysis and HbA1c
  • Good understanding of diabetes issues – including medication management and user involvement
