An exciting opportunity has arisen for a Community Diabetes Specialist Nurse to join the team at UHCW.

A Diabetes Specialist Nurse requires expertise in the management of diabetes in both hospital and community settings. The role is educational and advisory with the aim of providing specialist diabetes care to the person with diabetes, their carer and training for healthcare professionals.

The role involves working autonomously as well as within the multidisciplinary team to achieve targets set out within National guidelines.

A full UK driving license is essential for this post. Please note that an international driving license is not accepted.

Main duties of the job

  1. Accountable for own actions in accordance with the NMC Code of Professional Conduct and Scope of Professional Practice.
  2. Will contribute to corporate objectives, acting within local, trust and statutory guidelines and policies at all times.
  3. Act as a role model to nursing staff by delivering high quality care.
  4. To manage and organise a specific caseload of patients and prioritise referrals.
  5. To accept referrals from other health disciplines and refer patients to health professionals independently.
  6. To assist patients to achieve individualised goals of diabetes management to maintain their physical wellbeing.
  7. To advise medical and nursing staff on the most appropriate treatment for their patient based on clinical assessment and clearly defined policies and protocols.
  8. To convert patients to insulin therapy on an inpatient and outpatient basis.
  9. To advise patients on adjustment of their oral hypoglycaemic agents or insulin treatment to optimise control of their diabetes.
  10. To select, carry out and interpret the results of specific tests and take appropriate action.

Person Specification

Qualifications

Essential

  • oStudy at Level 6 (degree/ diploma) oNursing Degree or equivalent. oCurrent Professional Registration oEvidence of continuous professional development

Desirable

  • oPost registration validated course in diabetes oCounselling course/ module oAdvanced health assessment oNon-Medical Prescribing qualification. oTeaching and assessing qualification

Experience

Essential

  • oProfessional clinical experience in Diabetes speciality. oAbility to successfully manage people and change oHighly developed communication skills

Desirable

  • oTeaching experience. oExperience of writing / producing patient information. oExperience and evidence in research and audit.

Knowledge

Essential

  • oA knowledgeable clinician with the ability to apply evidence based practice. oA sound knowledge of professional policies and procedures relating to diabetes.

Desirable

  • oPreceptorship of new staff
