District Nurse Team Lead

Are you ready for a challenge and would like an opportunity to step up into the DN Team lead role?

An exciting Band 7 Community Nurse Team Lead opportunity has arisen with us at Carlisle & District ICC, covering a maternity leave.

There is an expectation that applicants hold the SPQ qualification or CSP Equivalent.

Main duties of the job

This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting. The role will also require you to provide clinical leadership and operational management of the community nurse team along with the management of the clinical caseload for the team.

ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of the local people.

We are able to provide an individually tailored preceptorship program to experienced nurses.

Person Specification

Qualifications

Essential

  • Current NMC registration 1st level registration
  • BSc/MSc/SPQ equivalent clinical nursing experience or advanced practise
  • Evidence of recent Continual Professional Development

Desirable

  • Mentorship Course or Teaching and assessing qualification or equivalent
  • Specialist chronic disease management course
  • Nurse Prescribing qualification (V150 or V300)

Knowledge and Experience

Essential

  • Significant post registration experience
  • Leadership or Team Management experience
  • Experience of service modernisation or change management

Desirable

  • Experience in primary or urgent care
  • Clinical Audit
  • Community working

Skills and Aptitudes

Essential

  • Demonstrate clinical knowledge in acute and chronic disease management
  • Knowledge of good clinical practice
  • Knowledge of current health and social care policies and legislation
  • Demonstrable knowledge of Clinical Governance
